Disclaimer- A mystery gift will be given to the first 100 early birds. Redemption of gift is based on a first come, first served basis.
- The mystery gift can only be redeemed and collected on the day of the event.
- Goodie bags are available in limited quantities and will be distributed on a first-come, first-served basis.
- The Organiser or Sunway Medical Centre, Sunway City Kuala Lumpur reserves the right to limit the number of goodie bags per main registrant and unable to guarantee availability for all attendees.
- You will be sent a confirmation email by 5 September 2024.
- The Organiser or Sunway Medical Centre, Sunway City Kuala Lumpur reserves the right to change these terms and conditions at any time without prior notice.
- This form is for pre-event submission purposes only to estimate attendance. All Attendees are still required to register with the e-mail confirmation notice on the event day to secure their slot.
- The personal data collected through the registration form/ portal and provided by the Attendee(s) for the purpose of this Event will be processed in accordance with the Personal Data Protection Act 2010 and the Personal Data Protection Notice of the Organiser (available at www.sunwaymedical.com). By participating in the Event, the Attendee(s) consents to the Organiser processing his/ her personal data, which includes, but is not limited to, their name, contact number and address, in a manner that permits the Organiser or authorised representatives to contact the Attendee(s) regarding matters related to the Event.
- A confirmation email will be sent to you by 5th September if your slot in the Batik Workshop is secured.
- The SunMed Health Carnival 2024 Batik Workshop ("Workshop") is organised by Sunway Medical Centre Sdn Bhd (199501012653 [341855-X]) ("Organiser" or "Sunway Medical Centre") and is open to all Attendees ["Attendee (s)"] aged 13 years and above. Children aged 12 and under can participate if their parents or guardians have registered for the Workshop. They will share one kit with their parents or guardians.
- The Workshop will be held on 8 September 2024 at the Tower D Lobby of Sunway Medical Centre, Sunway City Kuala Lumpur. Registration for the Workshop is FREE OF CHARGE, and no payment will be collected.
- The Organiser reserves the rights to withhold or disqualify any Attendee(s) participation in the event that they fail or refuse to comply with this Workshop's Terms & Conditions.
- Attendees are required to pre-register by providing their details in the “Registration Form” section on the website. However, this pre-registration does not guarantee a slot. Slots will only be confirmed on the event day upon the Attendee(s)’ presence and final registration at the event.
- The Organiser assumes no responsibility or liability for any damage, loss, harm or injuries resulting from participating in the Workshop.
- The Organiser reserves the right to use, disclose and publish any of the particulars /pictures/ artwork of the Attendees of the Workshop, as well as other personal particulars about them or their legal guardian(s), for any legitimate purpose as deemed appropriate, including but not limited to advertising and publicity purposes, without compensation to the attendees (or, where applicable, their authorised representatives).
- In the event that, for any reason, any aspect of this Workshop is not capable of running as planned, the Organiser reserves the right to amend, postpone or cancel the Workshop at its absolute discretion.
- By participating in this Workshop, all Attendees warrant that all statements and personal details given are true, accurate and complete. The respective parties are also taken to have read, understood and agreed to be bound by these Terms and Conditions, and accept all decisions by the Organiser as final and binding. The Organiser reserves the right, at its sole and absolute discretion, to change, amend, add or delete any of these Terms and Conditions at any time, which may be communicated through modes of communication deemed suitable by the Organiser, and without prior notice to the Attendee(s) or guardians of the Attendee(s), and the Attendee(s) and guardians of the Attendee(s) hereby agree to be bound by such changes.
- Failure or delay by the Organiser to enforce any provision of these Terms and Conditions shall not constitute a waiver of that provision.
- These terms and conditions shall be governed by and construed in accordance with the laws of Malaysia.
